Forms

We are requiring that all forms still be submitted electronically. Many of our staff will continue to work remotely and will not have access to paper forms submitted to our office. Please submit forms as email attachments to [email protected]; emails must be from a pdx.edu address.

  • Please ensure the form is complete with all required signatures before submission
  • Forms can only be submitted as PDFs; we cannot accept photographs of forms
  • We will accept Adobe Sign and DocuSign; we cannot accept script font in lieu of a signature
  • Forms can contain a combination of digital and wet signatures
  • One form per PDF; please do not submit multiple forms in one PDF file
  • If digital or wet signatures are not possible, we will accept email approval submitted with the form; the email must be from a pdx.edu address

ETD Access Form

The Survey of Earned Doctorates (SED) is online. Please register and complete the survey on the SED website. You must finalize the survey by clicking the "submit" button at the end. Additional information about the SED is available on our Thesis and Dissertation Information page (scroll to bottom of page).

Change of Term Form

The Change of Term form has moved to Slate and the .pdf Change of Term form is no longer necessary. The Slate training guide includes instructions for submitting a Change of Term form in Slate along with some details on the Change of Term process.
